How do I file a Medicare complaint (grievance)?

To file a Medicare complaint, you have several options depending on your issue. Call 1-800-MEDICARE (1-800-633-4227) to speak with a representative who can help you file your complaint directly. You can also file online by logging into your Medicare.gov account and selecting the complaint option under your plan details. For quality of care issues, contact your state’s Quality Improvement Organization (QIO), which handles complaints about hospital care, nursing home care, home health services, and other Medicare-covered services.

You can file complaints about Medicare providers, hospitals, doctors, health insurance plans, care quality, dialysis or kidney transplant services, and durable medical equipment issues. The Medicare Beneficiary Ombudsman can also assist with complex complaints that haven’t been resolved through other channels. Contact them at 1-800-MEDICARE and ask to be transferred.

Understanding the difference between complaints and appeals is important. Complaints address quality of care, customer service issues, waiting times, cleanliness, or staff behavior.
