The major improvements to Medicare drug coverage that began in 2025 continue in 2026. The $2,000 annual out-of-pocket cap for prescription drugs remains in place. Once you spend $2,000 out of pocket on covered Part D drugs in 2026, you won’t pay anything more for the rest of the year.
New for 2026, you can now choose to spread your prescription costs throughout the year with the Medicare Prescription Payment Plan. Instead of paying large amounts at the pharmacy counter, you can opt to pay in smaller monthly installments, making your drug costs even more manageable.
The old catastrophic phase remains eliminated. Previously, you had to pay five percent of drug costs after reaching a certain spending threshold. That coinsurance is gone for good.
Part D plans continue to cover a larger share of drug costs above the threshold, helping lower what beneficiaries pay overall.
